2022 Executive Summary
In its second year, Family Connects Pierce County expanded its reach, serving 486 families through postpartum nurse visits designed to strengthen caregiver well-being and connect families to vital resources. Nurses made over 600 referrals—13% for behavioral health and 14% for maternal and infant follow-up care. Six emergency referrals may have saved lives. The introduction of Baby Lounges provided safe, affirming spaces for 116 families, with attendees reporting reduced anxiety and stronger connections. The program also broadened its service definition to include all families who received any form of nurse support, not just completed home visits, reflecting a more inclusive measure of impact.

2021–2022 Annual Report
The first year of Family Connects Pierce County served 317 families and issued nearly 700 referrals through voluntary postpartum nurse visits. Launched to reduce child welfare involvement and fill postpartum care gaps, the program began at two hospitals and targeted families in three key zip codes. About 94% of families had at least one identified need, and many received support for material goods, healthcare access, or emotional well-being. Several cases involved emergency referrals. The year also piloted services for special populations, including those with postpartum depression or substance exposure. Insights from this launch phase informed deeper outreach, stronger infrastructure, and the development of Baby Lounges to enhance parent connection and mental health.
